At Brooklyn Care we strive to offer the best care suited for you or your loved one current health condition and needs.

We conduct a pre-admission evaluation (charged additionally) in order for us to understand your needs, and to ensure Brooklyn Care is the best suited facility.

Patients may be referred to another facility, should we not be able to offer the best suited care for your health condition. 

Kindly have the following documentation on hand to speed up the application process:

• Medical Aid
• Latest prescription and list of medication used on a daily basis
• Copy of ID document
